What reverse T3 elevation actually tells you clinically

Reverse T3 is one of the more misunderstood labs in functional practice, valuable when the clinical picture calls for it, misleading when it is ordered reflexively or read in isolation. It is worth knowing exactly what an elevated result does and does not mean.

What it is

Your body converts T4 into either active T3 or inactive reverse T3. Under stress, illness, inflammation, or caloric restriction, the conversion shifts toward reverse T3, a built-in metabolic brake that slows the system to conserve energy. Elevated reverse T3 is that brake showing up on a lab. It is a signal, not a diagnosis.

What it is telling you

A high reverse T3 points upstream, to a driver the body is responding to, most often chronic stress and elevated cortisol, systemic inflammation, undernutrition or aggressive caloric restriction, recent illness or surgery, or conditions like liver disease, kidney disease, and poorly controlled diabetes. The clinical value is not the number itself. It is what the number tells you to go looking for.

Why it matters in functional practice

This is the patient who feels hypothyroid, fatigue, low energy, sluggish metabolism, while TSH and even free T4 look acceptable. Reverse T3 can help explain the gap. You are no longer asking is the thyroid broken. You are asking what is driving this patient’s physiology to shift conversion, and that question leads to the root cause.

When to order it, and when not to

Reverse T3 is not a routine lab, and ordering it on everyone is not functional medicine, it is just more testing. It earns its place when a patient has persistent hypothyroid-type symptoms with a normal TSH and free T4, when you suspect a stress, inflammation, or undernutrition driver, or when you are distinguishing a conversion problem from a production problem. Outside of that reasoning, it adds cost and confusion, not clarity.

How to read it without being misled

Never interpret reverse T3 alone. It only means something next to free T3, the active hormone, and in the context of the patient’s story, their stress load, nutrition, recent illness, and other labs. Two patients with the same reverse T3 can need completely different plans.

The functional takeaway

Reverse T3 does not diagnose anything. It tells you the body has engaged its metabolic brake, and it sends you upstream to find out why. Ordered with justification and read in context, it is one of the more useful windows into why a symptomatic patient is not converting.
