The thyroid workup: testing thyroid physiology, not chasing thyroid numbers
Do not chase thyroid numbers. Understand thyroid physiology. The best functional clinicians do not simply diagnose hypothyroidism, they identify where in the thyroid system the patient is struggling, and they match the testing to that specific question. The thyroid is not one number, it is a system with five points where it can break down, and the workup follows the physiology.
Step one: Production
Is the gland making enough hormone?
This is where TSH and free T4 do their work, the pituitary signal and the gland’s actual output. This step is the conventional starting point, and for a screening question it is often enough. But production is only the first of five questions, and a normal TSH closes only this one.
Step two: Conversion
Is T4 becoming active T3?
The gland can produce adequately while the body fails to convert T4 into the active hormone the tissues use.
Free T3 answers this question, and reverse T3 has a selected role in patients where impaired peripheral conversion is suspected in the setting of chronic illness, inflammation, physiologic stress, or significant metabolic adaptation.
Reverse T3 is not a routine thyroid test. It answers a specific conversion question in carefully selected patients.
Step three: Autoimmunity
Is the immune system attacking the gland?
TPO antibodies and thyroglobulin antibodies answer this, and they often rise years before the TSH moves, identifying the early autoimmune patient while the standard screen still looks normal.
Hashimoto’s is an immune problem expressed in the thyroid, and naming it changes monitoring and counseling long before overt failure.
Step four: Nutrient support
Does the body have the substrates and cofactors for optimal thyroid function?
The thyroid cannot produce, convert, or effectively utilize hormone without adequate nutrient support.
Ferritin is one of the most commonly overlooked contributors to impaired thyroid physiology, particularly in menstruating women, but vitamin D, B12, folate, and, when clinically indicated, zinc and selenium also influence thyroid function.
The goal is not to order every nutrient on every patient. The goal is to identify deficiencies that explain why thyroid physiology is underperforming.
Step five: Physiology
What upstream factors are impairing thyroid function?
This is the functional step the others build toward, the recognition that thyroid dysfunction is often downstream of something else, stress and cortisol, inflammation, metabolic health and insulin resistance, sex hormones, gut health, sleep, medications, and lifestyle.
The thyroid often becomes the messenger rather than the root cause. Functional medicine asks why the thyroid is struggling before assuming the thyroid itself is the primary problem.
The match, applied to thyroid
Each step is a different clinical question, and the testing follows the question.
- If the issue is production, TSH and free T4 answer it.
- If the patient is symptomatic despite normal production, you are asking a conversion question, and free T3 with selected reverse T3 answers it.
- If you suspect early autoimmune disease, antibodies answer it.
- If the system looks adequate but underperforms, you ask the nutrient question.
- And when the thyroid keeps faltering despite an optimized panel, the question is physiologic, and you look upstream.
The functional takeaway
A thyroid workup is not a single number or even a single panel, it is a way of asking where in the system this patient is struggling. Production, conversion, autoimmunity, nutrient support, or upstream physiology.
When you match the laboratory evaluation to that specific question, testing becomes purposeful and far more likely to change care.
Do not chase thyroid numbers. Understand thyroid physiology, and test the step where the system is breaking down.
