The ferritin level that quietly stalls thyroid conversion

This is the patient whose thyroid labs look fine, TSH normal, free T4 and even free T3 acceptable, who still feels hypothyroid. Fatigue, hair shedding, cold intolerance, brain fog. Before you conclude the thyroid is not the problem, check the iron stores. Ferritin is one of the most overlooked drivers of thyroid symptoms, and it acts in a place the thyroid panel does not show you.

Why ferritin matters to the thyroid

Iron is required at three separate steps. It is a cofactor for thyroid peroxidase, the enzyme that builds thyroid hormone. It is needed to convert T4 into the active T3. And it is required for T3 to be used inside the cell, at the receptor. So low iron stores can impair production, conversion, and utilization all at once. A patient can have a normal panel and still be functionally hypothyroid at the tissue level, because the hormone is there but the iron needed to use it is not.

The number, honestly

Standard labs flag iron deficiency far too low, often only below 15 to 30, so a ferritin of 25 reads as normal while the patient is symptomatic. Functionally, concern begins earlier, around 50 and below, and for symptomatic thyroid patients the optimal target runs higher still. In one study of women with persistent hypothyroid symptoms despite thyroid medication, two-thirds saw their symptoms resolve once ferritin rose above 100. Treat these as functional anchors, not absolutes. A ferritin in the 30s or 40s in a symptomatic thyroid patient is worth your attention even though the lab called it normal.

The reverse T3 connection

Low ferritin can also push more T4 toward reverse T3, the inactive form that occupies T3 receptors without activating them. So iron deficiency can both lower active T3 and raise the inactive form that blocks it, a double hit the standard panel will not explain.

The caveat that protects you

Ferritin is an acute-phase reactant. It rises with infection, inflammation, liver disease, and metabolic dysfunction, which means a normal or even high ferritin in an inflamed patient can hide a true deficiency. Never read ferritin in isolation. Read it alongside the clinical picture and, when the story warrants, additional iron markers like serum iron, TIBC, and transferrin saturation.

When to check it

Not a reflexive add-on. It earns its place in the patient with hypothyroid symptoms and a normal thyroid panel, the patient still symptomatic on thyroid medication, or the patient whose history points to low iron, heavy menstrual periods, plant-based diet, GI issues, or pregnancy.

The functional takeaway

When the thyroid panel is normal but the patient is not, the answer is sometimes one step upstream, in the iron stores the thyroid needs to make, convert, and use its own hormone. Conventional ranges will call a low-normal ferritin fine. The functional move is to ask whether fine is good enough for this symptomatic patient.
